Resumen: | Las infecciones respiratorias recurrentes son frecuentes en la Parálisis Cerebral (PCI). La tos asistida manual (TAM) y mecánica (TM) persigue preservar las funciones del aparato respiratorio. Objetivo:. Evaluar la eficacia del tratamiento con vibraciones transtorácicas más insuflación-exuflación mecánica frente a las técnicas manuales, para inducir tos asistida en pacientes pediátricos con PCI grave o profunda. Método: 40 niños con PCI hospitalizados por infecciones respiratorias con edad media 3,7 años (rango 1-13) fueron asignados aleatoriamente a dos grupos de tratamiento: TM y TAM hasta 10 días o remisión. Resultados: la TM fue más eficaz en saturación de oxígeno (p=0,000), confort ventilatorio (p=0,001), número de sesiones (p=0,01) y reducción de síntomas (p=0,023). No existen diferencias en la frecuencia cardíaca y respiratoria. Conclusiones: la TM es más eficaz que la TAM para reducir los síntomas, mejorar el confort ventilatorio y normalizar la saturación oxihemoglobínica, logrando tales resultados en menos tiempo. ABSTRACT: Reoccurring respiratory infections are common in Cerebral Palsy (CP). Manually assisted cough (MAC) and mechanical assisted cough (MC) aim to preserve the functions of the respiratory system. Objective: To evaluate the efficacy of transthoracic vibrations more mechanical insufflation-exsufflation compared to manual techniques to induce assisted cough in pediatric patients with severe or profound PCI. Method: 40 children with CP hospitalized with respiratory infections, the average age is 3,7 years (ranging from 1-13) were randomly assigned to two treatment groups: MC and MAC up to 10 days or remission. Results: MC was more effective for oxygen saturation (p=0.000), comfort in ventilation (p=0.001), number of sessions (p=0.01) and reduction in symptoms (p=0.023). No differences were detected in the respiratory and cardiac frequency. Conclusions: MC is more effective than MAC in reducing symptoms, improving comfort and standardizing the ventilatory oxihemoglobine saturation, achieving these results in less time.
