- PublicationRestrictedBehavior and attitudes among Spanish general dentists towards the anticoagulated patient: a pilot study(Wiley, 2010-01-21) López Jornet, Pia; Camacho Alonso, Fabio; González Escribano, Myriam; Martínez Beneyto, Yolanda; Dermatología, Estomatología, Radiología y Medicina FísicaAim To determine the attitude of Spanish general dentists in relation to dental extraction in patients with heart valve prostheses subjected to acenocoumarol anticoagulation. Material and methods A telephone survey was made of Spanish general dentists, asking about the indicated approach in the case of performing dental extractions in patients with heart valve prostheses subjected to acenocoumarol (Sintrom®) anticoagulation. Results Of the 271 dentists answered, 175 were male (64.6%) and 96 were female (35.4%), with a mean professional experience of 20.17 ± 9.3 years. A total of 228 dentists (84.1%) indicated the need to refer the patient to a specialist for establishing the required approach, while 43 dentists (15.9%) did not consider such a measure to be necessary. Among this latter group of 43 dentists, 26 (60.5%) considered that acenocoumarol should be withdrawn or replaced by low-molecular weight heparin, while 17 (39.5%) were of the opinion that the anticoagulation regimen should not be modified. In relation to the international normalized ratio, 36 (83.7%) did not consider it necessary to request this parameter. As regards the prevention of endocarditis, 11 (25.6%) specified the need for prophylaxis, although only eight (72.7%) did so correctly. There were no statistically significant differences in behaviour in relation to either gender or years of professional experience. Conclusions This study identifies a lack of knowledge on the part of the dentists regarding the approach to dental extraction in patients with heart valve prostheses subjected to anticoagulation. Due educational measures therefore should be reinforced among these professionals.
- PublicationRestrictedDental students’ knowledge of and attitudes toward anticoagulation dental treatment: assessment of a one-day course at the University of Murcia, Spain(Wiley, 2012-04-01) Martínez Beneyto, Yolanda; López Jornet, Pia; Camacho Alonso, Fabio; González Escribano, M.; Dermatología, Estomatología, Radiología y Medicina FísicaThe aim of this study was to determine a group of Spanish dental students’ knowledge of and attitudes toward anticoagulated patients and clinical practice and to assess the need for additional education in this area. A survey with sections on demographics and knowledge of general coagulation, anticoagulation treatment, clinical practice, local hemostatic measures, and antibiotic prophylaxis was distributed to students before and after a one-day course on oral anticoagulation. The response rate was 67.1 percent (n=96). Of the respondents, 78.1 percent (seventy-five) were women. The students who benefited most from the course were third-year students since they had the lowest level of knowledge at the beginning of the course (p>0.05). Fifth-year students and postgraduates had a good initial level of knowledge (87 percent). This study identified a lack of knowledge among this group of Spanish dental students prior to the course, but participating in the course led to considerable improvement.

Estudio de 85 casos(Elsevier, 1999) García-Vázquez, Elisa; Gárgolas Hernández, Miguel de; García Delgado, Rosa; Fernández Guerrero, Manuel L.; MedicinaRESUMEN: Objetivo: Analizar la incidencia de neumonía por P. carinii después de suspender su profilaxis, en pacientes con sida en tratamiento antirretroviral de alta eficacia. Material y Métodos: Estudio prospectivo abierto de 85 pacientes con sida (linfocitos CD4 < 200 x 10 6 /1) y cuya cifra de linfocitos CD4 se incrementó por encima de 200 x 10 6 /1 después de terapia antirretroviral. En 79 casos la profilaxis era primaria y en seis secundaria. Resultados: La media de linfocitos CD4 al suspender la profilaxis fue de 343 x 10 6/1. El tiempo medio de seguimiento una vez suspendida la profilaxis fue de 358 días (límites: 93-1.487; mediana: 302). Ninguno de los enfermos ha desarrollado P. carinii o toxoplasmosis al cierre del estudio. Conclusiones: La profilaxis de infección por P. carinii puede ser suspendida en los enfermos que gracias a terapia antirretroviral intensiva alcanzan valores de linfocitos CD4 por encima d200 X 10 6/1.
