dc.description.abstractOBJETIVOS: - Determinar la correlación entre los valores SUV (SUVmax, SUVmean y SUVpeak) del 18F-FDG PET/TC basal, y el grado histológico según la clasificación de la FNCLCC en pacientes con sarcomas de partes blandas, y establecer un punto de corte que permita discriminar los sarcomas de alto grado de los de bajo grado. - Analizar la utilidad de los valores SUV en el 18F-FDG PET/TC basal y de seguimiento, como factores pronósticos relevantes en la supervivencia global y supervivencia libre de enfermedad, en pacientes con SPB. - Analizar la utilidad clínica de los parámetros de cuantificación metabólica basados en el volumen (volumen metabólico tumoral y tasa de glicólisis) del 18F-FDG PET/TC basal y de seguimiento, en pacientes con sarcomas de partes blandas. METODOS: Se realizó un estudio retrospectivo en pacientes con SPB (2011-2018), que contaran con informe de AP que incluyera los criterios de la FNCLCC, y 18F-FDG PET/TC basal. Se tuvieron en cuenta igualmente los estudios 18F-FDG PET/TC de seguimiento. Se comprobó la utilidad de los valores SUV y los parámetros de cuantificación metabólica basados en el volumen del 18F-FDG PET/TC basal como variables predictoras del grado histológico, utilizando curvas ROC, calculando el área bajo la curva con un IC del 95%. Para estudiar las posibles diferencias entre los valores SUV y los parámetros de cuantificación metabólica basados en el volumen y el grado histológico (FNCLCC), se realizó la prueba T-student. Se realizó un estudio de función de supervivencia mediante el método Kaplan-Meier, calculando la SG y la SLE. Para valorar la utilidad pronóstica de los valores SUV y los parámetros de cuantificación metabólica basados en el volumen del 18F-FDG PET/CT basal, se realizó un estudio de función de riesgo (Log-Rank). Se ordenaron todos los valores medidos en el estudio basal, y se estableció la mediana como punto de corte para crear dos grupos. Se elaboraron curvas pareadas para determinar si existían diferencias significativas entre las curvas de SG y SLE. Para evaluar la utilidad pronóstica de los valores SUV y los parámetros de cuantificación metabólica basados en el volumen del 18F-FDG PET/TC post tratamiento, se calculó la disminución porcentual de cada parámetro medido en el estudio de seguimiento con respecto al basal (%DISM), y se ordenaron dichos valores estableciendo la mediana como punto de corte para crear dos grupos. Se elaboraron curvas pareadas para determinar si existían diferencias significativas entre las curvas de SG y SLE. Todos los resultados se consideraron estadísticamente significativos para un nivel alfa <0.05. El análisis se llevó a cabo con el SPSS v.20.0. RESULTADOS: Se analizaron un total 83 pacientes (42 hombres y 41 mujeres). La media de edad al diagnóstico fue de 50,62 años. El 76% de las lesiones fueron de alto grado (grado 2 y 3 de la FNCLCC). El 40% de los pacientes presentaron metástasis en el estudio basal, de los cuales el 87.88% corresponden a SPB de alto grado. En el estudio basal, las medianas de los valores SUVmax, SUVmean, SUVpeak y la TGL fueron más altas en los SPB de alto grado que en los de bajo grado. Se estableció la utilidad de los parámetros metabólicos medidos en el estudio basal para discriminar los SPB de alto grado de los de bajo grado mediante curvas ROC, estableciendo como puntos de corte significativos los valores: SUVmax 3,9 [AbC 0,824 (IC del 95% 0,717–0,931)], SUVmean 2,5 [AbC 0,798 (IC del 95% 0,682 – 0,914)], SUVpeak 3,73 [AbC 0.817 (IC del 95% 0.708 – 0.926)], siendo el más significativo el valor SUVmax. El VMT y la TGL no permitieron establecer un punto de corte significativo. Se determinó la asociación entre el grado histológico y los parámetros metabólicos del 18F-FDG PET/TC basal mediante la prueba T-student, siendo significativa en los valores: SUVmax [p=0.04 (IC del 95% -10,12 a -1,93)], SUVmean [p=0.01 (IC del 95% -3,62 a -0,42)], y SUVpeak [p=0.006 (IC del 95% -8,35 a -1,44)]. No se encontró una relación significativa para el VMT (p=0.36) ni la TGL (p=0.34). Se realizó un estudio de supervivencia mediante el método Kaplan-Meier, obteniendo una media de SG 55,2 meses [error típico 4,4, (IC del 95% de 46,5 – 64)] y una SLE de 47,9 meses [error típico de 5,5, (IC del 95% 36,9 – 58,8)]. El estudio Log-Rank confirmó la utilidad pronóstica del 18F-FDG PET/TC basal para la SG en los valores: SUVmax [p=0.037 (IC 95% 12,5 – 69,4)], SUVpeak: [p=0.05 (IC 95% 13,1 – 68,8)], VMT [p=0.022 (IC 95% 7,8 – 74,1)], y TGL [p=0.022 (IC 95% 30,9 – 59)]. No hubo diferencias significativas en la SG para el SUVmean (p=0.141), ni en la SLE para los valores SUVmax (p=0,51), SUVmean (p=0,74), SUVpeak (p=0,60), VMT (p=0,91) y TGL (p=0,19). Al analizar los 18F-FDG PET/TC post tratamiento (29 estudios), el estudio Log-Rank determinó la utilidad pronóstica para la SG del %DISM en los valores: SUVmax [p=0,006 (IC 95% 15,7 – 42,2)], SUVmean [p=0,003 (IC 95% 11,6 – 32,33)], SUVpeak [p=0,006 (IC 95% 15,7 – 42,2)], VMT [p=0,006 (IC 95% 15,8 – 42,09)], y TGL [p=0,003 (IC 95% 11,9 – 32)]. En la SLE, solamente el %DISM del VMT demostró tener utilidad pronóstica [p=0,003 (IC 95% 4,7 – 13,2)]. CONCLUSIONS: - Los valores SUVmax, SUVmean y SUVpeak del 18F-FDG PET/TC basal son útiles para discriminar los SPB de alto grado de los de bajo grado, existiendo adicionalmente una relación significativa entre el grado histológico y los valores SUV. - El porcentaje de disminución de los valores SUVmax, SUVpeak, VMT y TGL en el 18F-FDG PET/TC post tratamiento, tienen valor pronóstico para la SG en pacientes con SPB. - El porcentaje de disminución de VMT en el 18F-FDG PET/TC post tratamiento, tiene valor pronóstico para la SLE en pacientes con SPB. - El 18F-FDG PET/TC es una herramienta útil en la estadificación y seguimiento de pacientes con SPB, con valor pronóstico en la SG y SLE.es
dc.description.abstractOBJECTIVES: - To determine the correlation between SUV values (SUVmax, SUVmean, and SUVpeak) measured on a baseline 18F-FDG PET / CT, and the tumor histological grade, according to the FNCLCC grading system, on patients with soft tissue sarcomas, and to establish optimal cut-off points to discriminate high-grade from low-grade sarcomas. - To analyze the usefulness of SUV values, measured on the baseline and follow-up 18F-FDG PET / CT, as relevant prognostic parameters in overall survival and disease-free survival for patients with soft tissue sarcomas. - To analyze the usefulness of the volume based quantitative metabolic parameters (metabolic tumor volume and total lesion glycolysis) measured on baseline and follow-up 18F-FDG PET/CT in patients with soft tissue sarcomas. METHODS: A retrospective study was conducted of patients diagnosed with STS between 2011-2018, who had anatomopathological confirmation that comprised the histological grades provided in FNCLCC classification as well as a baseline and follow-up 18F-FDG PET/CT. The usefulness of SUV values and the volume based quantitative metabolic parameters measured on baseline 18F-FDG PET/TC as predictor variables of the histological grade was evaluated, using ROC curves, calculating the area under the curve with an IC of 95%. A statistical analysis was carried out using the T-student test to search for significant differences between SUV values, volume based quantitative metabolic parameters and histological grade (FNCLCC). A survival function study was performed using the Kaplan-Meier method, calculating the overall survival (OS) and disease-free survival (DFS). A risk function study using the Log-Rank method was performed to determine the prognostic significance of SUV values and volume based quantitative metabolic parameters on the baseline 18F-FDG PET/CT. All measured values in the baseline study were ordered, and the median was used as the cut-off point to create two groups. Paired curves were deployed to detect statistically significant differences between overall (OS) and disease-free survival (DFS) between the two groups. To assess the prognostic significance of SUV values as well of the volume based quantitative metabolic parameters measured on the follow-up 18F-FDG PET/TC, the percentage decrease of each parameter obtained in the follow-up study with respect to the basal values (% DISM) was calculated. Then these values were arranged to determine the median, and use it as the cut-off point to create two groups. Paired curves were projected to detect statistically significant differences between the OS and DFS curves. All results were considered statistically significant with alpha level <0.05. The analysis was performed with SPSS v.20.0. RESULTS: 83 patients, 42 men and 41 women, were analyzed. The mean age at diagnosis was 50.62 years. 76% of the diagnosed STS were high-grade (Grade 2 and 3 FNCLCC). 40% of the patients presented metastasis in the baseline 18F-FDG PET / CT, out of which 87.88% were high-grade. The medians of total lesion glycolysis (TLG) and SUV values (SUVmax, SUVmean, SUVpeak) in baseline 18F-FDG PET/TC scored higher in high-grade than in low-grade STS. The usefulness of the metabolic parameters measured in the baseline study to discriminate high-grade from low-grade soft tissue sarcomas was established using ROC curves, setting the following values as significant cut-off points: SUVmax 3,9 [AUC 0,824 (95% CI 0,717–0,931)], SUVmean 2,5 [AUC 0,798 (95% CI 0,682 – 0,914)] and SUVpeak 3,73 [AUC 0.817 (95% CI 0.708 – 0.926)], being the SUVmax value the most significant. The metabolic tumor volume (MTV) and TLG parameters did not allow to define a significant cut-off point. The association between the histological grade and the metabolic parameters of the baseline 18F-FDG PET/TC was evaluated using the T-student test, and found significant in the values: SUVmax [p=0.04 (95% CI -10,12 a -1,93)], SUVmean [p=0.01 (95% CI -3,62 a -0,42)], and SUVpeak [p=0.006 (95% CI -8,35 a -1,44)]. No statistically significant relationship was found for the MTV (p-0.36) or TLG (p-0.34). A survival study was performed using the Kaplan-Meier method [OS 55.2 months (95% CI of 46.5 – 64)], [DFS 47.9 months (95% CI 36.9 – 58.8)]. The Log-Rank study confirmed the prognostic utility of the 18F-FDG PET/TC for OS: SUVmax [p-0.037 (95% CI 12.5 – 69.4)], SUVpeak: [p-0.05 (95% CI 13.1 – 68.8)], MTV [p-0.022 (95% CI 7.8 – 74.1)], and TLG [p-0.022 (95% CI 30.9 – 59)]. There were no significant differences in the OS for the SUVmean (p-0.141), or in the DFS for SUVmax (p-0.51), SUVmean (p-0.74), SUVpeak (p-0.60), MTV (p-0.91), and TLG (p-0.19). A Log-Rank study in the follow-up 18F-FDG PET/TC (29 studies) ascertained the prognostic utility for OS in the decrease percentage of: SUVmax [p-0.006 (95% CI 15.7 – 42.2)], SUVmean [p-0.003 (95% CI 11.6 – 32.33)], SUVpeak [p-0.006 (95% CI 15.7 – 42.2)], MTV [p-0.006 (95% CI 15.8 – 42.09)], and TLG [p-0.003 (95% CI 11.9 – 32) CI] . As for the DFS, only the MTV decrease percentage proved to have prognostic utility [p-0.003 (95% CI 4.7 – 13.2)]. CONCLUSIONS: - SUVmax and SUVpeak values of baseline 18F-FDG PET / CT are useful to discriminate high-grade from low-grade STS, with a statistically significant relationship between histological grade and SUV values. - The decrease percentage of SUVmax, SUVpeak, MTV and TGL values in the follow-up 18F-FDG PET/TC have prognostic value for OS in patients with STS. - The MTV percentage decrease in the follow-up 18F-FDG PET/TC has prognostic value for DFS in patients with STS. - The 18F-FDG PET/TC is a useful tool for staging and follow-up of patients with STS, with prognostic value in OS and DFS.es
