Título: Análisis de las reacciones psicológicas en los padres de niños ingresados en una unidad de cuidados intensivos neonatales

Resumen: Introducción El nacimiento de un recién nacido (rn) de alto riesgo (tanto prematuro como a término) que requiere de ingreso en la Unidad de Cuidados Intensivos Neonatal (UCIN) va a poner a los padres frente a una realidad nueva y desconocida para ellos, donde se les va a privar del vínculo materno/paterno-filial deseado durante largo tiempo. Además, como consecuencia del riesgo vital que puede ir aparejado al ingreso de su hijo, esta situación puede provocarles síntomas de estrés, ansiedad y depresión. Estos síntomas pueden interferir negativamente en la adaptación de los padres a la estancia de su hijo en la UCIN, imposibilitando un afrontamiento efectivo que les permita continuar manteniendo un vínculo adecuado y participar en su cuidado. Igualmente, puede ocurrir que en el momento del alta hospitalaria no se sientan capaces de proporcionar los cuidados que precisa, desempeñando un rol paterno inadecuado. No todos los padres sufren estas reacciones con la misma intensidad. Asimismo, su evolución a lo largo del tiempo de ingreso del rn también puede variar. Objetivos: 1. Explorar los niveles de estrés, ansiedad y depresión en padres de rn ingresados en una UCIN. 2. Identificar si variables sociodemográficas de los padres, así como variables clínicas relacionadas con la gestación y el neonato están relacionadas con los niveles de estrés, ansiedad y depresión que exhiben los padres. 3. Determinar la evolución de las reacciones psicológicas en ambos progenitores a lo largo del tiempo. Metodología Estudio descriptivo observacional longitudinal a lo largo de los primeros 6 meses de vida del rn realizado en la UCIN del Hospital Virgen de la Arrixaca (Murcia). Se realizaron mediciones en cuatro intervalos temporales: al ingreso en UCIN, al traslado a la unidad de Neonatología, previo al alta a domicilio y a los 6 meses de vida. El estrés, los síntomas de ansiedad y depresión se evaluaron con los cuestionarios Escala de estrés parental: UCIN (PSS: NICU), Escala de ansiedad estado – rasgo (STAI) y Escala de depresión de Beck II (BDI-II) respectivamente. Resultados La muestra total ha estado formada por 99 madres y 94 padres, con una edad media=34,1 y DT= 4,98 años. Con respecto a las variables psicológicas evaluadas, los resultados obtenidos muestran la presencia de estrés a nivel moderado (puntuación media en PSS: NICU=3,54, DT=1,32). Las subescalas del cuestionario que muestran puntuaciones más elevadas fueron Impacto en el rol parental y Aspecto y comportamiento del bebé. Con respecto a los síntomas de ansiedad, la puntación media en STAI para la subescala estado es 28,88 (DT=13,04), mientras que para la subescala rasgo es de 22,07 (DT=11,53). Estos datos indican que los progenitores presentan un nivel de ansiedad alto para la ansiedad estado y moderado para la ansiedad rasgo. La puntuación media en el cuestionario BDI-II=11,90 (DT=8,48) indica niveles mínimos de síntomas depresivos. Las variables sociodemográficas que mostraron una relación estadísticamente significativa (p<.05) con las variables psicológicas analizadas fueron el sexo, la edad, el nivel de estudios y el tipo de convivencia de los padres. De otro lado, algunas de las variables clínicas incluidas, tales como las semanas de gestación, tipo de parto, tipo de concepción…, también mostraron relación estadísticamente significativa. El análisis longitudinal de las variables depresión y ansiedad muestra un efecto significativo de la variable tiempo, observándose una reducción progresiva en los síntomas psicológicos desde la primera a la última fase de evaluación, no encontrándose diferencias de género. Conclusiones: Este estudio documenta las reacciones psicológicas que sufren los padres de los niños ingresados en estas unidades, y pone de manifiesto la necesidad de extender los cuidados del rn también a sus progenitores, para así favorecer el establecimiento óptimo del vínculo padres-hijos. ABSTRACT Introduction The birth of a high-risk newborn (both premature and full-term) requiring admission to the Neonatal Intensive Care Unit (NICU) will face parents with a new and unknown reality where they will be deprived of the long-desired parent-child bond. In addition, because of the life-threatening risk associated with their child's, this situation can lead to stress, anxiety and depression symptoms in the parents. These symptoms may interfere negatively in the adaptation of parents to their child’s stay in the NICU, thus making impossible an effective coping that would allow them to maintain an adequate bond and participate in their care. Likewise, it may happen that at the time of hospital discharge they do not feel able to provide the care the baby needs, playing an inadequate parent role. Not all parents suffer these reactions with equal intensity. Besides, their evolution during the newborn’s hospitalization can also vary. Aims: 1. Explore the levels of stress, anxiety and depression in parents of infants admitted to a NICU. 2. Identify whether sociodemographic variables of parents, as well as clinical variables related to gestation and the newborn are related with the levels of stress, anxiety and depression exhibited by parents. 3. Determine the evolution of psychological reactions in both parents over time. Methodology A longitudinal observational descriptive study during the first 6 months of life of the newborn that was carried out in the NICU of the Hospital Virgen de la Arrixaca (Murcia). Measurements were made at four time intervals: at the time of entry in the NICU; when transferred to the Neonatology unit; prior to discharge; and at 6 months of age. Stress, anxiety and depression symptoms were assessed using the Parental Stress Scale Questionnaire: NICU (PSS: NICU), State-Trait Anxiety Inventory (STAI) and Beck Depression Inventory-II (BDI - II) respectively. Results The total sample consisted of 99 mothers and 94 parents, with a mean age of 34.1 and SD = 4.98 years With respect to the psychological variables evaluated, the results obtained show the presence of moderate stress (mean score in PSS: NICU = 3.54, SD = 1.32). The subscales of the questionnaire that showed higher scores were Impact on parental role and Newborn appearance and behavior. Regarding anxiety symptoms, the mean score in STAI for the state subscale is 28.88 (SD = 13.04), while for the trait subscale is 22.07 (SD = 11.53). These data indicate that parents have a high level of anxiety for state anxiety and moderate for trait anxiety. The mean score in the BDI-II questionnaire = 11.90 (SD = 8.48) indicates minimal levels of depressive symptoms. The sociodemographic variables that showed a statistically significant relationship (p <.05) with the psychological variables analyzed were gender, age, level of education and type of cohabitation of parents. On the other hand, some of the clinical variables included, such as the weeks of gestation, type of delivery, type of conception ..., also showed a statistically significant relationship. The longitudinal analysis of the depression and anxiety variables shows a significant effect of the time variable, with a progressive reduction in the psychological symptoms from the first to the last phase of evaluation, not finding gender differences. Conclusions: This study documents the psychological reactions experienced by the parents of the children admitted to these units and highlights the need to extend the care of the newborn to their parents as well, thus favoring the optimal establishment of the parent-child bonding.
