Ortesis plantares rígidas conformadas y ondas de choque extracorpóreas en el tratamiento de la fascitis plantar.

Abstract

OBJETIVOS: La fascitis plantar es un proceso degenerativo doloroso y frecuente, con diversos tratamientos poco sustentados por la evidencia. Sí lo están la adaptación de ortesis plantares semirrígidas y las ondas de choque extracorpóreas (OCE). Queremos establecer los beneficios clínicos y funcionales para las actividades de la vida diaria (AVD) al acabar el tratamiento y a lo largo de tres meses de seguimiento cuando se asocian plantillas semirrígidas individualizadas al tratamiento de la fascitis plantar con OCE. Detectaremos y cuantificaremos además los efectos secundarios. METODOLOGIA: Estudio prospectivo aleatorizado. Hemos reclutado cuarenta pacientes diagnosticados de fascitis plantar. Treinta han sido tratados con ortesis asociadas a OCE, y diez con OCE exclusivamente. Hemos medido el dolor a la palpación en la tuberosidad interna del calcáneo y el dolor al comienzo de la marcha después de un periodo de reposo por medio de la Escala Analógico Visual (EVA). La funcionalidad para las AVD la hemos medido con el Cuestionario FAAM (Foot and Ankle Ability Measure). Hemos determinado los resultados al acabar el tratamiento, al mes y tres meses después. RESULTADOS: Ha mejorado el dolor en ambos grupos tras tres meses de seguimiento, aunque sólo se consigue que la mejoría sea progresiva a lo largo de esos tres meses cuando se asocian ortesis semirrígidas a OCE. La funcionalidad para las AVD sólo ha mejorado asociando ambos tratamientos. El efecto secundario más frecuente ha sido el dolor durante la administración de OCE, aunque en ningún caso ha sido causa de abandono de la terapia. Hemos detectado además un caso de Algodistrofia Simpático Refleja, que hasta ahora no había sido documentado en la literatura después del tratamiento con OCE.

ABSTRACT

OBJETIVES: Plantar fasciitis is a common and painful degenerative condition. There are commonly used treatments, but the evidence base underpinning these treatment strategies is often unknown. Customized functional Orthosis and Extracorporeal Shock Waves (ESW) are evidence based. We want to determine the clinical and functional daily life activity benefits once the treatment has finished and along the three-month follow-up when associating both therapies, and to detect the main side effects. METHODS: Prospective and randomized study. Forty patients with plantar fasciitis were treated. Thirty were treated with customized foot orthosis associated to ESW. Ten were treated with ESW only. We measured both pain upon palpation in the internal calcaneal tuberosity and pain in the first footsteps after resting by means of the ten-point Analogic Visual Scale. Functional Daily Life Activities were measured by the FAAM Questionnaire (Foot and Ankle Ability Measure). All measures were acquired when therapies finished, with follow-ups one month and three months afterwards. RESULTS: Pain improved in all patients after three months of follow-up, although that improvement was only evident throughout those three months when both treatments were combined. Likewise, function improved only when both treatments were associated. No relevant adverse events occurred in either intervention group. Pain was the most frequently secondary event while ESW were administered, although it was not necessary to abandon treatment in any case. We detected one single case of Complex Regional Pain Syndrome. This event was unknown in the literature after treatment with ESW.
