dc.description.abstract | La evaluación del pronóstico y estratificación del riesgo en el síndrome coronario agudo sin elevación del segmento ST (SCASEST) con troponina negativa, sigue siendo un desafío. Los principales hallazgos de esta tesis fueron: i) Sólo IL-6 y PCRus presentan concentraciones aumentadas en pacientes de bajo-moderado riesgo con SCASEST y troponina T estándar negativa con respecto a pacientes con cardiopatía isquémica estable y a sujetos sanos; ii) los niveles de IL6, PCRus y PIGF se asociaron independientemente con eventos adversos a los 6 meses de seguimiento; iii) una estrategia multimarcador, definida como 2 ó 3 biomarcadores elevados con papel pronóstico, mostró ser el mayor predictor independiente de eventos adversos en estos pacientes; iv) la adición de esta estrategia multimarcador a la escala TIMI, mejoró la estimación del riesgo de los pacientes de bajo-moderado riesgo con SCASEST y troponina T estándar negativa, y reclasificó correctamente un tercio de estos pacientes. We sought to explore the prognostic value of biomarkers among low-moderate risk patients with NSTE-ACS and troponin T negative, and to evaluate whether a multimarker strategy could improve the predictive performance of TIMI risk score. The main findings were: i) only the IL-6 and hsCRP showed higher levels in these patients respect to patients with stable ischemic heart disease and healthy subjects; ii) the levels of IL-6, hsCRP and PIGF were independently associated with adverse events at 6 months follow-up; iii) a multimarker strategy, defined as 2 or 3 elevated biomarkers with a prognostic role, proved to be the greater independent predictor of adverse events in low-moderate risk patients with NSTE-ACS and troponin T negative; iv) the addition of this multimarker strategy to TIMI risk score improved the estimation of risk in low-moderate risk patients with NSTE-ACS and troponin T negative, and correctly reclassify one third of these patients. | es |
