CHANGES OF PLATELET FUNCTION IN EXPERIMENTAL BILIARY CIRRHOSIS: INTRACELLULAR CALCIUM HOMEOSTASIS Introduction: In liver cirrhosis, several haemostatic abnormalities occur, mostly episodes of bleeding, but there is a 9-20 % risk of thrombosis. Some extrinsic and intrinsic factors are altered in these patients and their role in the development of these alterations is not completely known. Objetives: To investigate the role of homocysteine (Hcy), bile acids, nitric oxide (NO) and folic acid on platelet function of rats with experimental cirrhosis. Methodology: We have used platelets of rats with bile duct ligation (BDL), to measure aggregation, Ca2+-intracellular levels and its regulation ([Ca2+]i), reactive oxygen species-(ROS) and the expression of P-selectin. Conclusions: LCB rats without ascites show greater platelet aggregation, mobilization of [Ca2+]i, ROS production and expression of P-selectin and less capacitative calcium entry (ECC). In platelets of LCB with ascites, many of these alterations are absent. The absence of NO and/or presence of bile acids reduce ECC in platelets of LCB animals. Folic acid can reverse the hyperaggregation, overexpression of P-selectin, reduce the release of Ca2+ stores and normalize ROS production. Keywords: Bile duct ligation (BDL), homocysteine (Hcy), nitric oxide (NO), reactive oxygen species (ROS), capacitative Ca2+ entry (ECC), intracellular calcium ([Ca2+]i), deoxycholic acid (DC) and chenodeoxycholic (CDC).

At present, alcohol consumption is a major socioeconomic and health problem worldwide, being the main reason for the development of chronic liver diseases. The development of liver cirrhosis is a disease whose unique solution in many cases is liver transplantation. The mechanisms involved in the development of this disease activate an inflammatory response mediated by the innate immune system through NK cells, and other cells. The mechanisms involved in development include an inflammatory response mediated by NK cells, among others. NK cells participate in this process through membrane receptors such as KIR receptors, which regulate the immune response through the bond of HLA-C molecules. Objective: To know the main indications to liver transplantation in the Region of Murcia, including alcoholic liver cirrhosis, and main clinical and sociodemographic characteristics, as well as the influence of KIR genes and HLA-C molecules in their development. Patients and methods: It is an observational, longitudinal and retrospective descriptive study. The population of Chapters 1 was composed of 1026 patients who underwent liver transplantation at the Hospital Universitario Virgen de la Arrixaca between 1988 and 2013. From these amount, 281 male patients with alcoholic cirrhosis were selected for Chapter 2; from those, 213 did not associate the viral infection, and 68 did associate it. As controls subjects, 319 liver donors were used. A number of clinical, analytical and radiological data were identified and the KIR genes from all transplanted patients were determined by two types of polymer chain reaction techniques, in addition to the KIR and HLA-C genomes of each individual. Results: The sample of chapter 1 had a mean age of 51.1±0.38 years, from which 73% were men and 27% women. Alcoholic cirrhosis was the most frequent (38.4%; 76.6% no virus associated and 23.4% with virus), followed by virus cirrhosis (24.1%). Survival of alcoholic cirrhosis was the highest (81.2% a year), being lower in patients with alcoholic cirrhosis with virus associated (74.2% a year). The sample of chapter 2 had a mean age of 53.63±0.495 years. Biochemical parameters showed high values of total bilirubin (3.17±0.30mg / dl). GOT, GPT and GGT showed values 3 times higher than normal, as well as alkaline phosphatase, which was significantly increased in patients with alcoholic cirrhosis with viral infection compared to those who did not associate virus. This difference was statistically significant (P=0.007). The INR presented mean values of 1.44±0.05, slightly higher than normal values. Prothrombin activity (%) showed levels below normal (58.93±0.94). KIR2LD2/S2+ gene was significantly increased (P=0.013) in the healthy population (63.3%) compared to the total of patients (53.0%), and KIR2DS5+ gene was significantly increased (P=0.033) in patients (35.2%) compared to the healthy population (27%). Analysis of HLA-C epitopes showed that the number of patients without C1 ligand was significantly increased in the patient population relative to the healthy population (P=0.007). C2C2 genotype was significantly higher in the patients (23.5%) than in the controls (15.3%) (P=0.015). Conclusions: Alcoholic cirrhosis is the main cause of liver transplantation, being more frequent in men. The absence of KIR2LD2/S2+ gene and the presence of KIR2DS5+ gene may predispose to the development of alcoholic cirrhosis. Analysis of HLA-C seems to indicate that the C2C2 genotype may predispose to the development of alcoholic cirrhosis.

Bacteremia in patients with hepatic cirrhosis is a frequent complication associated with increased mortality; The knowledge of the predisposing factors and prognoses of bacteremia in this group of patients would improve their approach and prognosis. The main objective is to describe the epidemiological, clinical and laboratory characteristics of patients with hepatic cirrhosis and bacteremia. Based on this knowledge, the characteristics of cirrhotic patients with and without bacteremia are analyzed to identify potential predictive factors of bacteremia and later the prognostic factors associated with mortality in patients with cirrhosis of the liver and bacteremia are analyzed. METHODS: This is a prospective and observational cohort study of episodes of bacteremia in cirrhotic patients admitted to the Santa Lucía Hospital in Cartagena during the period from 1st December 2008 to 31st December 2015, a total of 278 episodes of liver. Of the total number of bacteriemias, 81 episodes of bacteremia were included, corresponding to 75 patients (5 patients had more than one episode of bacteremia) and 188 cirrhotic patients who entered the study period without bacteremia. A descriptive study, a comparative analysis of the patients with and without bacteremia and an analysis of the prognostic factors associated with mortality were carried out. The comparison between study variables was performed using Pearson's X² or Fisher's exact test for qualitative variables and Student's T for quantitative variables and a statistical significance of p ≤0.05 was considered. A multivariate analysis was performed to assess the statistically independently associated factors with mortality including variables that were significant in the bivariate analysis or those that were not of clinical interest. Mortality was assessed for this analysis at year 1 of the episode. In addition, a Cox regression survival analysis was performed. A Cox regression analysis was performed to estimate the independent effects of possible survival predictors that had been significant in univariate analyzes. Finally, we performed a Kaplan-Meyer curves analysis of survival of patients with bacteremia of our cohort. The SPSS version 15.0 for Windows was used for the statistical analysis. RESULTS AND CONCLUSIONS: In the descriptive analysis the following results were highlighted: the incidence of bacteremia in cirrhotic patients in our center was 7.2 / 100,000 admissions / year compared to 6.9 / 100,000 admissions / year in the general population. 52% of the cases were community bacteremias and the most frequent focus were urinary (31%), respiratory (23%) and abdominal (17%). Only 17% of the patients with bacteremia had a CRP≥10 mg / dL, compared to 68% with PCT> 1ng / mL. 91% of the patients had a MELD-Na> 15 points, with a Pitt≥3 in 65%; Being the mortality during the episode was of 9.8%, at 30 days of 33% and the year of 62%. In the multivariate analysis we found the active alcohol consumption (OR 4.6, 95% CI 2.97-10.87), MELD disease severity scales> 15 points (OR 4) as predictors of bacteremia in cirrhotic patients , 15%, 95% CI 1.7-10.13 and MELD-Na ≥ 15 (OR 5.5, 95% CI 1.98-15.24), elevation of biomarkers (CRP> 5 mg / dl and PCT> 1 Ng / ml), OR 7.48, IC95% 3.1-18.05 and OR9, IC95% 3.6-22.54, respectively) as well as being a vesical catheter as a previous invasive procedure (OR 2, 05, 95% CI 1.09-4.95). In the multivariate analysis we found as independent prognostic factors associated with higher mortality of the cirrhotic patient with bacteremia a value of PCT> 1 ng / mL (OR 9.17, 95% CI 2.33-36.11) and MELD severity scales > 15 points (OR 5.37, 95% CI 1.04-27.58), MELD-72 hours> 30 points (OR 4.07, CI 95% 3.31-22.08) and Pitt index ≥ 3 OR 4.97, IC 95% 1.3- 19.05). In the multivariate analysis we found the presence of bacteremia (OR 1.91, CI 1.07-2.47) and a value of the MELDNa severity scale as prognostic factors associated with higher mortality in the global cohort (n = 269) > 30 (OR 4.2, 95% CI 1.94-14.87).

PhD Thesis: UTILITY OF BIOMARKERS IN NON-INVASIVE ASSESSMENT OF LIVER FIBROSIS IN PATIENTS WITH CHRONIC HEPATITIS C AND OF NONALCOHOLIC FATTY LIVER DISEASE IN OBESE PATIENTS The overall objective is to evaluate the clinical utility of biomarkers hyaluronic acid (HA), aminoterminal peptide of procollagen type III (PIIINP) and tissue inhibitor of metalloproteinase-1 (TIMP-1), individually and combined by the algorithm ELF, in the assessment of liver fibrosis in patients with chronic hepatitis C and nonalcoholic fatty liver disease (NAFLD) in obese patients undergoing bariatric surgery respectively. This thesis comprises a cross-sectional observational study involving two groups of patients, one composed of 107 patients diagnosed with chronic hepatitis C and another group of 57 obese patients with suspected NAFLD underwent bariatric surgery. Study parameters understood clinical variables, parameters derived from the radiology study and laboratory variables. In both groups of patients, direct fibrosis biomarkers HA, PIIINP and TIMP-1, and ELF algorithm were evaluated. Serum concentrations were determined in the overall population and in subgroups of patients, and also their association with different clinical variables, radiology study and other laboratory variables. As reference standard, the Arfi technique was used in patients with chronic hepatitis C and liver biopsy in obese patients. The main conclusions were:  Patients with Chronic Hepatitis C: 1- The values of ELF algorithm and direct biomarkers HA, PIIINP, and TIMP-1 were higher in patients with significant fibrosis and in those who had ultrasound signs of chronic liver disease. 2- HA and ELF demonstrated clinical efficacy in identifying significant fibrosis and provided the best results of all variables studied. Furthermore, TIMP-1 and ELF proved the most effective variables in identifying cirrhosis. 3- Combination of ELF algorithm with clinical variables and indirect biomarkers is a valid and more effective option than either variable individually in identifying significant fibrosis. In identifying hepatic cirrhosis, the most effective option is the combination of ELF algorithm with clinical variables, indirect biomarkers and s ultrasound signs of chronic liver disease. The use of these algorithms would be very convenient in assessing the degree of hepatic fibrosis in patients with chronic hepatitis C.  Obese patients with suspected NAFLD 1- The values of ELF and HA were significantly higher in the group of patients who had in biopsy, in addition to steatosis, inflammatory infiltration, hepatocyte ballooning, Mallory´s bodys or fibrosis, and in hypertensive patients. On the other hand, patients with metabolic syndrome had higher values of ELF. 2- HA biomarker, ELF and Fibrometer algorithms and shear wave velocity showed clinical efficacy in the evaluation of chronic liver disease in the study population. ELF algorithm was, among these, the variable that showed a better combination of sensitivity and specificity. 3- The combination of direct fibrosis biomarkers and Arfi technique represent a valid option and more effective than the use of single biomarkers or techniques for identifying patients with NAFLD and early steatohepatitis or fibrosis, so that its use would be very convenient in the evaluation and stratification of the increasing number of patients with suspected disease.
