    Efecto de las técnicas de sellado alveolar con injerto epitelial libre vs matriz colágena porcina en los procedimientos de preservación alveolar

    Introducción: Los procedimientos de preservación alveolar (ARP) son técnicas predecibles que se utilizan para minimizar tanto la reabsorción ósea como el colapso de los tejidos blandos que se producen tras la exodoncia y facilitar, así, la colocación de implantes en la posición tridimensionalmente ideal. Objetivo: El objetivo de este ensayo clínico fue comparar y analizar los cambios volumétricos y lineales que se producen en los tejidos tras la realización de dos tipos de preservación alveolar. Material y Métodos: Se realizó un ensayo clínico en el que se incluyeron pacientes que requirieron exodoncias en zonas no molares. Grupo de prueba: 10 alveolos se rellenaron con mineral óseo bovino desproteinizado con colágeno al 10% (DBBM-C), y se sellaron con una membrana de colágeno porcina bicapa (MC). Grupo de control: se rellenaron 10 alveolos con DBBM-C y se sellaron con injerto epitelial libre autólogo (IEL). Se hizo un análisis del cambio volumétrico a los 3 y 6 meses de la cirugía mediante superposición de modelos estereolitográficos (.stl) y se utilizó un stent acrílico hecho a medida como referencia para las mediciones lineales de la cresta alveolar. Los análisis estadísticos se realizaron mediante t-test para muestras, test de Mann-Whitney, t-test pareado y test de Wilcoxon. Resultados: El cambio medio volumétrico que se produjo a los 6 meses fue de -37,2 ± 45,01 mm3 para el grupo control y de -13,78 ± 26,32 mm3 para el grupo test. No hubo diferencias estadísticamente significativas entre grupos (T-Test p=0.173). La media de los cambios horizontales que se produjeron en el tejido blando, durante los 6 meses de cicatrización, a 1 mm (D7), 3 mm (D8) y 5 mm (D9) del margen gingival fue de -2,5 mmm ± 1,03 mm, -1,89 ± 1,04 mm y -2,18 ± 2,27 mm respectivamente para el grupo control y de -1,58 ± 0,96 mm, -0,67 ± 1,08 mm y -0,69 ± 0,84 mm respectivamente para el grupo test. Aquí sí hubo diferencias significativas en los cambios que se produjeron a los 3 y a los 5 mm del margen gingival cuando se compararon los grupos control y test (T test p= 0.02 y M-W p=0.044). En la papila mesial (D1), el cambio medio para el grupo control fue de 0,7 ± 0,82 mm y de 1.1 ± 0,88 mm para el grupo test. En el punto medio del margen vestibular (D2), se registraron cambios de 0,6 ± 1,17 mm para el grupo control y de 0,9 ± 0,74 mm para el grupo test. En la papila distal (D3), los cambios fueron de 1,1 ± 0,74 mm para el grupo control y de 1 ± 0.94 mm para el grupo test. Finalmente, en el punto medio del margen gingival palatino los cambios registrados fueron de 1,9 ± 1,52 mm para el grupo control y de 1,2 ± 1,03 mm para el grupo test. Los cambios verticales en los tejidos blandos no mostraron diferencias significativas entre grupos. La media de los cambios óseos verticales en la cresta ósea vestibular (D5) fue de 0,7 ± 1,77 mm en el grupo control y de 0,9 ± 1,45 mm en el grupo test. En la cresta ósea palatina (D5) se registró un cambio medio de 1,3 mm ± 0,67 mm para el grupo control y de 1,2 ± 1,14 mm para el grupo test. No se encontraron diferencias estadísticamente significativas cuando se compararon los cambios entre ambos grupos (D5: M-W p= 1.00; D6: M-W p=0.427). Conclusiones: En general, las diferencias obtenidas en cuanto a cambios volumétricos y lineales son pequeñas entre los grupos estudiados. La aplicación de un sustituto de tejido blando xenogénico porcino como material de sellado alveolar tras la exodoncia, resulta ser segura y mantiene el volumen del alveolo de manera similar a la aplicación de un injerto epitelio-conectivo.

    The efficacy of a deproteinized bovine bone mineral graft for alveolar ridge preservation: a histologic study in humans

    Background: Alveolar ridge preservation (ARP) following tooth extraction plays a vital role in maintaining ridge dimensions and supporting subsequent implant therapy. Objectives: This study histologically and radiographically evaluates the efficacy of techBiomat bone®—a deproteinized bovine bone mineral (DBBM)—for alveolar ridge preservation (ARP), comparing the results of bone formation, residual graft particles, and nonmineralized tissue to those of spontaneous healing in human tooth sockets. Methods: A split-mouth study was conducted to evaluate the radiographic and histologic outcomes in human sockets with and without ARP. Results: A significant improvement in bone fill was observed compared to untreated sockets. Radiographically, 87% of the treated sockets demonstrated more than 75% bone fill, whereas only 7% of the untreated sockets did. Histologically, the percentage of new bone formation was greater in treated sockets (42%) than in untreated sockets (25%). The findings also highlighted a lower proportion of nonmineralized tissue in grafted sites, suggesting improved healing over spontaneous healing. The residual graft material in the treated sockets had a moderate resorption rate, with almost complete replacement by the host bone after six months. The use of techBiomat bone® demonstrated promising results, with a resorption rate conducive to optimal bone regeneration, with less than 9% residual graft material remaining after six months. Conclusions: This study supports the efficacy of techBiomat bone® graft material for ARP, highlighting its potential in maintaining ridge volume. Further studies with larger sample sizes are needed to confirm these findings.
